TSHR

促甲状腺激素受体(TSHR)是一种位于甲状腺滤泡细胞膜上的糖蛋白受体,全称为甲状腺刺激素受体。它负责感知促甲状腺激素(TSH)或自身抗体的结合,从而调控甲状腺激素(T3/T4)的合成与分泌。TSHR通过复杂的信号网络,在人体代谢、生长发育和能量平衡中发挥核心作用。近年来,TSHR被证实与多种自身免疫性甲状腺疾病及甲状腺癌密切相关,使其成为生物医药领域的研究热点。
TSHR的异常激活或失调与多种疾病有关。例如,Graves病患者体内产生自身抗体激活TSHR,导致甲状腺激素过量分泌和甲亢,并可能发展为Graves眼病,引起眼部肌肉肿胀。与之相反,桥本甲状腺炎患者体内存在阻断性抗体,抑制TSHR功能并引发甲状腺功能减退。此外,约80%的分化型甲状腺癌呈现TSHR过度表达,通过激活MAPK和PI3K/AKT通路促进肿瘤增殖及转移。基于TSHR的分子特性,放射性碘治疗已成功应用于甲状腺癌靶向治疗。
针对TSHR功能异常引发的疾病,已形成多维度靶向治疗格局。例如,CAR-T细胞疗法通过基因工程改造T细胞靶向TSHR,实现特异性杀伤;重组TSHR类药物如Thyrogen可增强甲状腺细胞摄碘能力,赛诺璟提升rhTSH半衰期,提高术后清甲成功率;单/双抗药物如GensSci098单抗降低游离T4水平并抑制眼眶成纤维细胞IL-6分泌,双抗药物VBS-102创新性构建TSHR和IGF-1R双靶点阻断机制,抑制多条通路活性,改善眼球突出。融合蛋白与基因疗法如VTC-G90降低甲状腺激素水平,基因编辑疗法缩小甲状腺体积,为治疗甲状腺相关疾病提供了新的方向。
